Physicals

To participate in sports, a current physical and immunization record must be on file with your child's health office. The exam must be dated within one year from the start date of the selected sport.

Physicals can be uploaded to the FinalForms registration platform or delivered to the school health office:

Fall

All athletes must have a current physical examination on file with their school health office. The physical exam must be dated August 2025 or later to be valid for the 2025 fall season.

Winter

 All athletes must have a current physical exam on file with their school health office. The physical exam must be valid for day one of the 2025-26 Winter season.

Spring

All athletes must have a current physical exam on file with their school health office. The physical exam must be valid for day one of the 2026 Spring season.
