Physicals

To participate in sports, a current physical and immunization record must be on file with your child's health office. The exam must be dated within one year from the start date of the selected sport.

Immunization Requirements

Physicals can be uploaded to the FinalForms registration platform or delivered to the school health office:

  • Amherst Central High School- 4301 Main Street, Amherst, NY 14226, Attn: Health Office. Fax- 716-836-4972; phone 362-8165
  • NOTICE - FROM JUNE 29 - AUGUST 21, 2026 - AMS Physicals will be faxed to the HS fax at 716-836-4972
     Amherst Middle School- 55 Kings Highway, Amherst, NY 14226, Attn: Health Office. Fax- 716-836-0193; phone 362-7104

Fall

All athletes must have a current physical examination on file with their school health office. The physical exam must be valid for day one of the 2026 fall season.

Winter

 All athletes must have a current physical exam on file with their school health office. The physical exam must be valid for day one of the 2026-27 Winter season.

Spring

All athletes must have a current physical exam on file with their school health office. The physical exam must be valid for day one of the 2027 Spring season.
