FinalForms Sport Signup Directions

Amherst Central Schools Athletic Department uses FinalForms, an online forms and data management service, to allow parents and guardians to safely complete and sign athletic participation forms for your child. FinalForms saves data from season-to-season and year-to-year and also pre-populates information wherever possible, for each child, saving you time.  You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.

We are asking that ALL parents of athletes use FinalForms. Paper forms are no longer being accepted to register your child for sports participation. When registering, follow the prompts to create your account, create your student's and sign your forms.  For step-by-step directions,  Parent Playbook.
